According to German daily newspaper Bild the horrific Love Parade incident in Duisburg, Germany, wasn’t so much an unforeseen accident, but a direct failure to follow safety guidelines. As 21 people died and 500 were crushed to injury, police blocked off what should have been an emergency exit:
